The Mexico Health Declaration Form is a mandatory international questionnaire that all passenger leaving and entering Mexico is required to complete that includes the below mentioned details:

flight and arrival destination. What is your actual physical condition now? What diseases have you had in the past? What are the conditions of your family (if any)? This information is required on the arrival health questionnaire.

Your health declaration form will ask where you would like to enter Mexico once you have decided to travel to Mexico. The most popular choice is Cancun as it is considered to be one of the safest cities in the world. All you have to do is follow the detailed instructions on how to complete this questionnaire and then wait for at least 3 hours in order to complete it and hand it over to the authorities at the airport.

It is very important to fill in the health declaration form accurately. It can be very difficult for the authorities to get all the details if they haven't captured everything at the airport yet. The officials at the airport might also have difficulties getting hold of you so they will simply ask you where you were last night and last week. They will use security cameras to monitor your departure and arrival movements.

Mexico has an active traveler crime system which makes it important for travelers to carry their Mexico Health Declaration Form when traveling outside of Cancun. You should keep the sheet with you at all times so that you can provide the authorities with any relevant information. This document is required to prove that you have not been infected by contagious diseases. This vaccination is required by law in Mexico and is carried out by most people who travel outside of Mexico.

The document proves that you have received the immunization within a certain period of time and the name of the doctor who administered the shot. Keep the sheet handy so you can give the immigration authorities any pertinent information about your stay in the country. It might be possible that you will be asked to provide further information when entering the country. This is only a procedure you should take if instructed by a doctor at an immigration checkpoint.
